Well, How Did We Get Here? The Current State of Pediatric Endocrinology

Objective:
To describe factors leading to the development of current workforce challenges in pediatric endocrinology and to propose solutions.
Study Design:
The author made use of 32 years of experience as chief of pediatric endocrinology at a large academic medical center, review of fellowship match and workforce data from the American Board of Pediatrics, and review of compensation data from the Association of American Medical Colleges.
Results:
Although many find pediatric endocrinology a fulfilling career, workforce shortages lead to increased workloads, and relatively low remuneration and excessively long training requirements discourage entry into the field.
Conclusions:
Potential solutions include value-based remuneration supported by robust funds-flow agreements with hospitals, streamlined training requirements, and utilization of informatics to support better targeted referrals and effective comanagement of selected patients with primary care physicians.
