Annular atrophic lichen planus (AALP) is a rare subtype of lichen planus. It is distinguished from other variants by the reduction of elastic fibers in the superficial dermis and the presence of annular skin lesions, which are characteristic features of this variant. AALP often exhibits a chronic course and poor response to treatment. The rare case presented here may provide valuable insights for dermatologists.

A case of a 57-year-old male diagnosed with generalized annular atrophic lichen planus is reported. Based on the clinical features and histopathological examination, a final diagnosis of generalized annular atrophic lichen planus was made. No significant abnormalities were found in the patient's baseline condition. Following the diagnosis of AALP, the patient was subjected to combination oral therapy with low-dose methylprednisolone (16 mg/day) and hydroxychloroquine (0.4 g/day).

A rapid resolution of the skin rash was observed two weeks after the initiation of treatment. During the clinical follow-up period of six months, no significant recurrence of the rash was noted.

This case suggests that oral corticosteroids combined with hydroxychloroquine may be an effective treatment option for generalized annular atrophic lichen planus.
