Torpedo Maculopathy Complicated by Macular Neovascularization and Secondary Multiple Evanescent White Dot Syndrome

Purpose:
To report a case of torpedo maculopathy complicated by choroidal neovascularization (CNV) followed by secondary multiple evanescent white dot syndrome (MEWDS).
Methods:
Case report.
Results:
A 28-year-old male was referred for acute vision loss in his right eye. Fundus examination revealed a torpedo-shaped lesion temporal to the fovea. Optical coherence tomography (OCT) revealed a pigment epithelium detachment (PED) with overlying subretinal hyper-reflective material (SHRM), flow signal on OCT-angiography as well as hyper fluorescence on fluorescein angiography, consistent with CNV. More peripheral to the macula, indocyanine green angiography revealed multiple late-phase hypo fluorescent spots being hyper-autofluorescent on fundus autofluorescence and exhibiting ellipsoid zone disruption on OCT, supporting the diagnosis of secondary MEWDS.
Conclusion:
This is the first reported case of torpedo maculopathy complicated by both CNV and MEWDS. This case study exemplifies how congenital macular anomalies can progress to neovascular and inflammatory complications.
