Abstract:
This article presents our experience in managing a case of a persistent infected nonunion of the proximal ulna that failed to heal after a free vascularized fibular graft. Free vascularized bone grafting is one of the best treatments for infected nonunion, but multiple factors must be addressed to achieve bone union. It is crucial to target septic, biological, and mechanical factors systematically using a step-by-step approach, even when faced with a worst-case scenario after a challenging surgery such as a free vascularized fibular graft.
