Integration of liquid chromatography-tandem mass spectrometry into total laboratory automation: technical challenges

Abstract:
The clinical laboratory is undergoing a paradigm shift toward the integration of liquid chromatography-tandem mass spectrometry(MS) into Total Laboratory Automation workflows. While historically relegated to specialized satellite units due to manual pre-analytical requirements, recent technological advancements have enabled the development of fully automated, 'sample-to-result' MS modules. This review provides an authoritative analysis of the technical integration of systems such as the Roche cobas® i 601 and Shimadzu CLAM-2040 in core laboratories. We examine the role of intelligent middleware and artificial intelligence-assisted algorithms in ensuring standardized data validation and quality control tracking and real-time operational monitoring. Furthermore, we evaluate the clinical impact of this integration on diagnostic accuracy for low-concentration analytes in endocrinology and therapeutic drug monitoring. By transitioning from batch-based processing to continuous automated streams, MS establishes itself as a robust, high-throughput tool for routine clinical diagnostics.
