Afferent loop obstruction presenting as obstructive jaundice.

    Summary

A rare case of obstructive jaundice was caused by gastric pouch cancer 25 years post-surgery for peptic ulcer disease. This finding suggests an independent pancreatic sphincter may exist, preventing pancreatitis despite biliary dilation.

    Area of Science:

    • Gastroenterology
    • Surgical Oncology

    Background:

    • Peptic ulcer disease often requires surgical intervention, such as gastric surgery.
    • Afferent loop obstruction is a known complication following certain gastric surgeries.

    Observation:

    • A patient presented with obstructive jaundice, a rare complication.
    • The cause was identified as a carcinoma of the gastric pouch.

    Findings:

    • The gastric cancer developed 25 years after initial peptic ulcer surgery.
    • Despite significant biliary tree and gallbladder dilation, no pancreatitis was observed.

    Implications:

    • This case highlights a late-onset complication of gastric surgery.
    • The absence of pancreatitis suggests the presence of an independent pancreatic sphincter, potentially the sphincter of Boyden.
