Abstract:
Biologic optimization of rotator cuff tears is perhaps the next frontier for improving outcomes after arthroscopic repair. A lack of clarity regarding multiple variables when it comes to platelet-rich plasma formulation and application limits the usefulness of these products. Recent literature helps provide clarity. Optimization of platelet-rich plasma with leukocyte-poor formulations while maximizing platelet concentration, along with intratendinous insertion, appears to be the best practice for improving retear rates and patient-reported outcomes. Although the improvements are not clinically substantial, we now have a pathway to attain meaningful clinical results with biologic augmentation.
