From discovery to proof: a Translation Evidence Map for precision psychiatry

Abstract:
Precision psychiatry aims to improve individual-patient decisions by measuring clinically meaningful heterogeneity, not by privileging any one data type, biomarker, or analytical method. This Personal View proposes a Translation Evidence Map that stages claims, from Level 0 (defined use case or discovery-positioning statement) to Level 6 (post-deployment monitoring), linked to measurement reliability, generalisable inference, decision value, safe delivery, and proportionality to risk, clinical need, and inaction. The Map does not replace Grading of Recommendations Assessment, Development and Evaluation (GRADE), biomarker qualification, prediction-model, regulatory, or roadmap frameworks; it identifies the highest defensible claim across components of unequal maturity and the next evidence step for compound precision claims. The Personal View discusses examples spanning inflammatory and circuit-based depression stratification, psychosis relapse monitoring, and suicidality monitoring, illustrating how the Map makes claim maturity, stopping points, stakeholder roles, and trade-offs important to patients explicit.
