Caring Without the Body: Semantic Displacement From Embodied Nursing to Artificial Intelligence

Abstract:
This paper examines an ontological question raised by the growing use of artificial intelligence (AI) in clinical care: what happens to concepts such as clinical intuition, suffering and presence when their origin shifts from the embodied, mortal nurse to a machine with no body and no history of vulnerability? Using critical conceptual analysis combined with reflexive engagement with clinical vignettes drawn from the author's own nursing experience, we check these vignettes against the established nursing-knowledge literature for referential adequacy. We argue that AI can reproduce the outward form of care-alerts, affective language and monitoring-without sharing the embodied history from which that form first acquired meaning for the person receiving it. We call this gap 'semantic displacement' and propose an asymmetry-of-cost criterion for judging when the gap matters clinically: displacement is most consequential where the human act, unlike its AI equivalent, costs the carer something. At the same time, we reject any suggestion that embodied human care is therefore uniformly superior or safe by default; documented failures of compassion in nursing history, and the evidenced effects of fatigue, burnout and bias on clinical judgement, show the embodied source of care is itself imperfect and unevenly distributed. The paper's contribution is accordingly not a defence of an idealised human nurse against a deficient machine, but an argument for a hybrid model in which AI absorbs monitoring and data-heavy labour so that scarce nurse time is protected for the embodied work AI cannot perform-while that freed time is itself protected from the workload pressures already eroding human care.
