Abstract:
A 44-year-old man developed otogenic sigmoid sinus thrombosis with cerebellar abscess, hydrocephalus, and recurrent posterior fossa infection. Staged neurosurgical drainage and canal-wall-down mastoidectomy achieved source control. Cultures were negative. Anticoagulant exposure was perioperative/early postoperative. Despite persistent venous non-opacification, he was discharged improved, afebrile, without neurological deficit, and with remitted otoliquorrhea.
