Telmisartan: distinct pharmacological features, partial PPAR-γ modulation, and pleiotropic therapeutic potential

Abstract:
Telmisartan, a selective angiotensin II type 1 (AT1) receptor antagonist, is widely used in the management of hypertension by selectively blocking AT1 receptor mediated signaling, thereby attenuating the deleterious effects of excessive renin angiotensin aldosterone system (RAAS) activation without directly inhibiting ACE2, AT2 receptors, or other components of the renin angiotensin system. This results in reduced vascular resistance, decreased aldosterone secretion, and improved cardiovascular and renal protection. This review outlines the broadened therapeutic scope of telmisartan, highlighting its established pleiotropic mechanisms and emerging therapeutic applications beyond its primary antihypertensive indication. Accumulating data indicate that it influences metabolic homeostasis by enhancing insulin responsiveness and regulating lipid dynamics, suggesting potential utility in conditions associated with metabolic imbalance. In addition, telmisartan demonstrates the capacity to modulate inflammatory pathways and counteract oxidative stress, contributing to improved vascular integrity and functional stability. Compared with other ARBs, telmisartan is distinguished by its partial PPARγ agonistic activity, prolonged elimination half-life, high lipophilicity, and sustained AT1 receptor blockade, which may provide additional metabolic and vascular effects. However, these pharmacological distinctions do not consistently translate into superior clinical outcomes across all therapeutic settings, and direct comparative evidence remains limited. Emerging evidence also points toward its involvement in interconnected cellular signaling networks, extending its relevance in complex disease mechanisms. Collectively, these findings support the concept that telmisartan exerts diverse biological effects, positioning it as a versatile therapeutic agent in modern clinical practice with implications that extend beyond traditional RAAS inhibition.
