Abstract:
In a recent retrospective cohort study by Wang et al., the perioperative hemodynamic variables were found to be associated with short-term graft function in pediatric renal transplant recipients, especially mean arterial pressure (MAP) before and after the graft reperfusion. However, a set age-based MAP threshold does not necessarily reflect the individual renal allograft perfusion needs. The time spent below a target MAP may not be equivalent between patients, as the MAP threshold may change depending on the recipient and donor blood vessel characteristics, and the baseline hemodynamics, as well as the donor-recipient size relationship. Future research with personalized hemodynamics targets could shed light on the effects of MAP control during surgery on early renal graft function.
