Abstract:
This case describes a neonatal truncus arteriosus repair using an 8 mm ring-reinforced, valved polytetrafluoroethylene conduit for right ventricular outflow tract reconstruction via the 'double-dunk' technique. The diagnosis was established prenatally in a patient with familial history of the disease. Anatomically, both pulmonary arteries arose adjacent to each other from the posterolateral common trunk; there was a mildly regurgitant quadricuspid truncal valve and an accidental finding of an intramural course of both coronary arteries. Clinically, the neonate presented with mild cyanosis and pulmonary overcirculation. The postoperative course was complicated by ventilator-associated pneumonia requiring prolonged ventilation. Discharge echocardiography showed good biventricular function, unobstructed conduit flow and mild truncal valve regurgitation. At the 6-month follow-up, the patient remains clinically well.
