Abstract:
Thanks to the rapid development of modern science and medicine, limb reconstruction can and should be taken to the next level. The author proposes the concept of "maximum limb reconstruction", which pursues not merely function but the maximal normalization of structure, morphology, and function, leveraging advances in prosthetics, artificial intelligence, and exoskeleton technology to enhance limb preservation. For the three pathological states-defect, infection, and deformity-sequential strategies are outlined: ① mutilating injuries are managed in three phases (early revascularization, mid-term wound coverage and infection control, late length restoration and functional reconstruction); ② osteomyelitis treatment must prioritize radical debridement over reconstructive techniques; ③ severe deformities require active limb salvage treatment, long-term follow-up, and recognition of natural remodeling potential. Ultimately, limb reconstruction surgeons should integrate modern technology with holistic patient needs-physical, psychological, and sexual-to continuously improve outcomes, recognizing that the native limb, however imperfect, holds irreplaceable value for human identity and well-being.
