Background:
Awake subglandular breast augmentation offers rapid recovery but requires effective perioperative analgesia. This study evaluated a standardized multilayer fascial plane anesthetic protocol for Feather Touch technique.
Methods:
In this prospective study, 66 women underwent primary subglandular breast augmentation between January and November 2025 under light intravenous sedation with ultrasound-guided interpectoral, pectoserratus, serratus anterior, and subcutaneous infiltration.
Results:
All procedures were completed without conversion to general anesthesia. Early recovery was uneventful in most patients. Four minor postoperative events were recorded: double-bubble deformity, bottoming out, palpable margins, and superior malposition. No major complications occurred. Lightweight implants were associated with improved early positional stability.
Conclusions:
A standardized multilayer fascial plane protocol can be safely integrated into awake subglandular breast augmentation using the Feather Touch technique. Matching anesthetic delivery to subglandular dissection supports intraoperative comfort and early recovery, while implant weight may influence early stability.
Level Of Evidence Iv:
Breast Surgery. This journal requires that authors assign a level of evidence to each submission to which Evidence-Based Medicine rankings are applicable. This excludes Review Articles, Book Reviews, and manuscripts that concern Basic Science, Animal Studies, Cadaver Studies, and Experimental Studies.
