    Summary

    The acetylator phenotype does not significantly impact dihydralazine therapy effectiveness or side effects in hypertensive patients. Further research is needed to fully understand dihydralazine metabolism.

    Background:

    • The acetylator phenotype, determined by N-acetyltransferase enzymes, influences drug metabolism.
    • Dihydralazine is an antihypertensive medication whose metabolic pathways may be affected by acetylation status.

    Purpose of the Study:

    • To investigate the influence of the acetylator phenotype on dihydralazine therapy in hypertensive patients.
    • To determine if acetylation status affects dihydralazine dosage requirements, plasma concentrations, or adverse effects.

    Main Methods:

    • Studied 21 hypertensive patients on chronic dihydralazine treatment.
    • Measured plasma dihydralazine concentrations 6 hours post-dose.
    • Classified patients into slow and fast acetylator groups.

    Main Results:

    • No significant differences were observed between slow and fast acetylators regarding dihydralazine dose, plasma concentrations, dose/concentration relationship, or side effects.
    • Antinuclear antibodies were detected in five patients, four of whom were fast acetylators.
    • The lowest dose/concentration relationship was noted in a patient with impaired renal function.

    Conclusions:

    • The acetylator phenotype appears to be an unimportant factor in dihydralazine therapy for hypertension.
    • Further investigation is required to fully elucidate the metabolism of dihydralazine.
