Rethinking the Gut-Bone Axis: Diet, the Gut Ecosystem, and Skeletal Health

Purpose Of Review:
The gut-bone field has expanded beyond its initial focus on the gut microbiota to include microbial metabolism, epithelial barrier integrity, immune and endocrine signaling, and host-microbial interactions. This review uses a functional gut ecosystem framework to integrate these mechanisms and examine how diet and other exposures influence skeletal health.
Recent Findings:
No consistent microbial signature of osteoporosis has emerged across populations or experimental models. Instead, recent studies indicate that skeletal responses are associated with functional outputs of the gut ecosystem, including nutrient bioavailability, microbial co-metabolic products, barrier integrity, and host sensing. Gut-derived signals may act directly on bone or be further processed by the host. Diverse interventions, including whole foods, probiotics, postbiotics, and targeted therapies, can converge on these functions. Bone responds to the integrated biological signals generated by the gut ecosystem rather than microbial composition alone. Defining these functions and individual responder phenotypes may provide new approaches for preserving skeletal health.

Role of Vitamins in Maintaining Bone Health
Vitamin A
Vitamin A is involved in the process of bone remodeling. Retinoic acid, the active metabolite of Vitamin A, has nuclear receptors in osteoblasts and osteoclasts, which are involved in bone remodeling.
Vitamin B12
Vitamin B12 acts as a cofactor during the formation of osteoblast-related proteins, such as osteocalcin. Vitamin B12 plays a role...
Bone Disorders
Bone deposition is also affected by the levels of sex hormones like estrogen and testosterone that promote osteoblast activity and bone matrix synthesis. When the level of these hormones decreases due to aging, it causes a reduction in bone deposition. As a result, bone resorption by osteoclasts...
Essential Minerals for Bone Health
Calcium and Phosphorus
Calcium is a critical component of bones, especially in the form of calcium phosphate and calcium carbonate. Since the body cannot make calcium, it must be obtained from the diet. However, calcium cannot be absorbed from the small intestine without...
