Abstract:
AbstractWhile general-purpose multimodal and language models have made progress across many medical tasks, many clinically important problems require a precise understanding of anatomy, imaging physics, and disease-specific context that is not available in internet-scale public data. In this issue of NEJM AI, Yang et al. report the TopCoW challenge, a public benchmark for topology-aware circle of Willis segmentation on computed tomography (CT) and magnetic resonance (MR) angiography. The study provides paired CT and MR angiography scans, voxel-level labels for 13 arterial components, virtual reality-assisted expert annotations, external validation, and downstream clinical tasks including fetal-type posterior cerebral artery classification and aneurysm localization. The lesson of TopCoW extend beyond those that concern the circle of Willis. The future of medical AI will not be built by generalist models alone. It will require clinically grounded datasets, shared benchmarks, multimodal learning, and annotation systems that make expert knowledge easier to capture.
