[Remifentanil PCA for Labour Pain Management - The Obstetric Anesthetists' Association (OAA) Guidelines as the Basis
Abstract:
The new guidance from the Obstetric Anaesthetists' Association (OAA) provides an evidence-based framework for the safe use of remifentanil patient-controlled analgesia (PCA) during labour. Remifentanil is not a substitute for neuraxial analgesia, but an important alternative when neuraxial techniques are contraindicated, technically impossible or declined. Key safety requirements include patient-controlled bolus administration, a two-minute lockout interval, continuous pulse oximetry and uninterrupted 1:1 observation by trained staff, with immediate access to oxygen, naloxone and ventilatory support. Compared with conventional systemic opioids such as pethidine, remifentanil offers better analgesia, but its use requires strict organisational and monitoring standards.

Since epidural anesthetics can be infused through an epidural catheter, all types of drugs, including short-acting ones, can be administered. Chloroprocaine and lidocaine are examples of short and long-duration anesthetics, respectively. Bupivacaine...
