Abstract:
Migration of temporary anchorage devices (TADs) into the maxillary sinus is rare and may cause inflammation or further displacement. A 17-year-old female developed recurrent pain after bilateral infrazygomatic crest TADs placement for anterior retraction. Initial cone-beam computed tomography (CBCT) showed a ~10-mm TAD along the medial wall of the right maxillary sinus with mucosal thickening. Follow-up CBCT demonstrated posterosuperior migration toward the orbital floor. Because the device was located high and difficult to access directly, it was removed through endoscope assistance within a ~3-mm transoral bony window. This case illustrates that displaced TADs may continue to migrate, and serial CBCT is valuable for monitoring position. Endoscope-assisted transoral retrieval may provide accurate localization in restricted areas while minimizing unnecessary bone removal.
