Combating the Opioid Crisis Through Stewardship in Pediatric Orthopedics: Using Quality Improvement Methodology to
Jennifer Weiner1, Jessica Holstine2
1Jennifer Weiner, Pediatric Nurse Practitioner, Department of Orthopedics, Nationwide Children's Hospital, Columbus, OH.
Background:
Rates of opioid misuse and deaths continue to climb despite efforts aimed at improving prescribing practices. Providing guidelines for opioid use in pediatric orthopedic surgery is an opportunity to enhance safety.
Aim:
Increase the percentage of patients who are prescribed the recommended doses of opioids for their procedure for patients discharged by Orthopedics from 75.8% to 90%.
Methods:
The Plan-Do-Study-Act (PDSA) model helped implement the interventions. Patients included in the project are those admitted to the Orthopedic Surgery service undergoing surgery.
Interventions:
Novel interventions include using the electronic medical record, providing new prescribing guidelines, and using a badge buddy.
Results:
Opioid dosages decreased from 8.15 to 4.43. Compliance with guidelines increased from 58.9% to 93.1%.
Conclusion:
Using a guideline for prescribing opioids after surgery has proven to be effective with the interventions. Combating the opioid crisis demands innovative and novel strategies to improve patient safety and manage postoperative pain.
