Background:
The safety of same-day discharge (SDD) following transcatheter aortic valve implantation (TAVI) is a well-established strategy in patients undergoing the transfemoral approach. The feasibility of SDD in patients undergoing transaxillary TAVI has not been reported.
Case Summary:
We present a 76-year-old male patient who underwent successful elective transaxillary TAVI. He had severe aortic stenosis, normal left ventricular function, and a background of diabetes, chronic liver disease, and right above-knee amputation. The access site was managed with 2 ProGlide devices with excellent hemostasis. There were no significant electrocardiogram changes postprocedure. Comprehensive postdischarge surveillance was provided, including a welfare telephone call on the following day.
Discussion:
To our knowledge, this is the first case in the literature of SDD following transaxillary TAVI. Such a strategy requires appropriate patient selection, meticulous procedural planning, rigorous postdischarge monitoring strategy, and can only be considered in highly selected cases.
