Objective:
Desai et al (2024) reported that 88.2% of 17 surveyed US payers would consider covering and managing drugs packaged with a novel large-volume on-body delivery system (OBDS) but not how individual payers would operationalize that coverage. This follow-on study used patent-pending, per-respondent voice-intelligence analysis to identify those mechanisms.
Methods:
Recordings of 11 of the 17 Desai 2024 interviews (93.7 million of the original 227 million covered lives) were analyzed using VOICE (Beyond Physician), a commercial voice-intelligence platform that scored six payer dimensions per respondent as a fixed-weight composite of transcript sentiment (60%), acoustic emotion (25%), and direct acoustic features (15%). Findings were reconciled with Desai 2024.
Results:
Three cross-file findings replicated: (1) a drug-centric coverage framework (5 of 11, plus 2 partially) helps explain why high feature ratings in Desai 2024 were not decisive for coverage; (2) 10 of 11 payers indicated that patient preference data alone is insufficient for coverage decisions without paired outcomes correlation; and (3) site-of-care enforcement was identified through five distinct mechanisms in five of six regional managed care organizations. Reconciliation found all eight directly comparable Desai 2024 findings consistent with the subset, with dissenting respondents reported where present.
Conclusion:
Voice-intelligence re-analysis of the same recordings was consistent with all directly comparable Desai 2024 findings and added per-respondent operational depth. Per-respondent analysis identified operational mechanisms, such as five site-of-care enforcement approaches, that the aggregate analysis did not report. The approach is applicable to other recorded qualitative stakeholder interview datasets where per-respondent reasoning adds value.
