Abstract:
Communication is a core clinical competence that directly impacts treatment adherence and patient outcomes. Although excellent communication, coaching, and educational frameworks have been developed, they often operate in isolation, leaving clinicians to navigate between disparate approaches without a unifying reference. The TOGETHER Model is introduced in this study as a conceptual proposal that integrates and synthesizes established evidence-based communication practices and professional coaching methods into a coherent and operational framework for patient-centered care, further grounded in recognized psychological theories. TOGETHER Model translates theoretical constructs into eight practical domains: "trust building and relationship," "open communication," "goal setting," "explanation," "treatment options and shared decision-making," "humanity," "education and empowerment," and "review and follow-up." Each domain is associated with practical professional actions and essential competencies, providing clinicians with structured yet flexible guidance that can be integrated across diverse care settings and patient journeys. The model is designed to be modular and scalable, supporting its use in medical education and continuing professional development. Operational applications include structured training curricula, simulation-based assessments, AI-enhanced digital platforms, and organizational toolkits for implementation and change management. Beyond education, the model offers a pathway for embedding communication competence into accreditation systems and quality standards, thereby aligning professional development with broader institutional and policy goals.

Managed Care System:
The managed care system is designed to control the cost while maintaining the quality of care. The patient's care from admission to discharge is planned by the primary care provider or the case manager, also known as the gatekeeper. In a managed care system, the number of care providers is limited...
Health Information Technology and Healthcare Information System
Health Information Technology, commonly called HIT, integrates advanced information systems and technology in healthcare settings. Its primary functions include:

Physicians
The physician's primary responsibility is to diagnose illness and direct the medical or surgical treatment of the condition. The authority to admit patients to a healthcare agency or institution and practice care within that setting is granted to physicians by the healthcare agency or institution itself.
