Introduction:
Etrasimod is an oral sphingosine-1-phosphate receptor modulator approved for the treatment of moderate to severe ulcerative colitis. Its targeted mechanism, once-daily dosing and oral formulation make it an important addition to the therapeutic landscape. However, its safety profile and optimal clinical positioning require careful evaluation.
Areas Covered:
This narrative review examines the pharmacology, efficacy and safety of etrasimod in ulcerative colitis, drawing on evidence from the OASIS, ELEVATE UC and ENLIGHT UC trial programmes, open-label extension studies, meta-analyses, pharmacovigilance reports and emerging real-world cohorts. The practicalities of prescribing and monitoring are also covered. A literature search was conducted in MEDLINE to identify appropriate studies.
Expert Opinion:
Current evidence supports etrasimod as a generally well-tolerated treatment option, with low rates of serious adverse events, serious infections, opportunistic infections and malignancy which do not differ substantially from placebo. Key safety considerations include first-dose bradycardia, atrioventricular conduction abnormalities, macular oedema, hepatic enzyme elevations, and lymphopenia, all of which necessitate appropriate screening and monitoring. Etrasimod may be particularly useful in patients with moderately active disease and limited prior advanced therapy exposure, whilst its reassuring safety profile and low risk of serious infection make it an attractive candidate for use in advanced combination therapy.

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
