Background:
Successfully completing quality improvement initiatives and reliably evaluating their impact are persistent challenges for healthcare systems. This study describes the implementation of a structured, system-level quality improvement framework across a multi-hospital network. The framework's design, project progression indicators, and the reliability of a standardized internal evaluation process are reported.
Methods:
We implemented a structured quality improvement framework across 8 hospitals in Saudi Arabia. The approach utilized standardized support, guided templates, and continuous mentorship, anchored to submission for a formal quality improvement award. A three-stage evaluation process assessed submissions using standardized rubrics with interrater reliability analysis.
Results:
Of 127 submitted projects, 50 completed the primary development cycle. Following initial screening, 26 projects advanced to formal judging with interrater reliability ranging from 0.667 to 0.900 across 12 evaluation criteria, with good reliability for 10 of 12 criteria. Thirteen projects progressed to final presentations with interrater reliability ranging from 0.392 to 0.880 across seven weighted categories. Five projects were selected as award recipients. High-performing entries addressed patient safety, clinical documentation optimization, interdepartmental communication, and process efficiency.
Conclusions:
In this descriptive program evaluation, a structured, internally led QI framework linked to institutional goals and transparent evaluation criteria was accompanied by project progression to defined milestones, and the internal evaluation process produced generally consistent ratings across most domains. Causal effects were not assessed.
