    • Surface electrocardiograms (ECG) may not reveal subtle atrioventricular (A-V) conduction abnormalities.
    • His bundle electrograms are crucial for diagnosing infra-Hisian conduction disturbances.

    Observation:

    • A case presented with ventricular parasystole and transient right bundle branch block during a His bundle electrogram.
    • No pre-existing A-V conduction disturbances were evident on the surface ECG.

    Findings:

    • His bundle electrogram revealed latent infrahisian conduction changes.
    • These changes indicate a susceptibility to developing right bundle branch block.

    Implications:

    • Induced right bundle branch block during procedures like right heart catheterization is possible.
    • These conduction abnormalities may contribute to the mechanism of ventricular ectopic activity.
