    Background:

    • Arsenic poisoning can cause severe hematologic abnormalities, including megaloblastic anemia.
    • Previous studies have reported hematologic complications but lacked direct evidence of arsenic in the bone marrow.

    • A patient with severe arsenic poisoning presented with megaloblastic erythropoiesis and other blood/bone marrow abnormalities.
    • Ultrastructural analysis revealed significant nuclear aberrations in bone marrow cells.

    Findings:

    • Electron probe analysis detected arsenic within bone marrow spicules.
    • These findings provide direct evidence of arsenic's presence in the bone marrow milieu.
    • Arsenic exposure is linked to megaloblastic anemia and direct toxicity to DNA synthesis.
