    Ulcerative colitis involving the terminal ileum, known as backwash ileitis, may increase the risk of small bowel cancer. This case study observed premalignant changes alongside multiple carcinomas in the colon and ileum.

    Background:

    • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
    • Backwash ileitis refers to inflammation of the terminal ileum secondary to UC.
    • The association between backwash ileitis and small bowel malignancy is not well-established.

    Observation:

    • A patient with long-standing ulcerative colitis and backwash ileitis presented with multiple carcinomas.
    • Carcinomas were found in both the colon and the ileum.
    • Premalignant mucosal changes, characterized by basal cell proliferation, were observed adjacent to and distant from cancerous sites.

    Findings:

    • The co-occurrence of premalignant and malignant changes in both the colon and ileum in this case is noteworthy.
    • This case highlights the potential for small bowel involvement in UC-related carcinogenesis.
