    Background:

    • Caroli disease is a rare congenital disorder characterized by cystic dilatation of the intrahepatic bile ducts.
    • Diagnosis in pediatric patients can be challenging due to its rarity and varied presentations.

    Observation:

    • Two pediatric cases of Caroli disease were identified.
    • One case was diagnosed intraoperatively using operative cholangiography.
    • The second case was diagnosed non-surgically via percutaneous transhepatic cholangiography.

    Findings:

    • While several imaging modalities can suggest Caroli disease, percutaneous transhepatic cholangiography (PTC) proved definitive.
    • PTC allows for direct visualization of the biliary tree, clearly demonstrating the characteristic cystic dilatations.

    Implications:

    • Percutaneous transhepatic cholangiography should be considered the gold standard for diagnosing pediatric Caroli disease.
    • Accurate and timely diagnosis is crucial for appropriate management and preventing complications such as cholangitis and portal hypertension.
