    Background:

    • Progressive muscular dystrophy (PMD) can affect cardiac function.
    • Early detection of cardiac involvement is crucial for managing PMD patients.

    Purpose of the Study:

    • To investigate systolic time intervals (STIs) in patients with different types and severities of PMD.
    • To assess the utility of STIs in identifying latent left ventricular (LV) dysfunction.

    Main Methods:

    • Indirect STIs, including pre-ejection period (PEP) and left ventricular ejection time (LVET), were measured at rest and during isometric handgrip exercise.
    • Seventy patients with PMD (Duchenne, limb girdle, facioscapulohumeral) were categorized by severity (mild vs. severe).
    • Patient data were compared to normal subjects.

    Main Results:

    • Severe Duchenne type PMD patients showed prolonged PEP, shortened LVET, and increased PEP/LVET ratio at rest compared to controls.
    • The PEP/LVET ratio increased during and after handgrip exercise in severe Duchenne type PMD.
    • Limb girdle and facioscapulohumeral types did not exhibit significant STI differences.

    Conclusions:

    • Resting and exercise STI measurements may aid in clinically recognizing subclinical LV functional impairment in PMD.
    • STI analysis is particularly valuable for identifying cardiac involvement in severe Duchenne muscular dystrophy.
