    Summary

    Two cases of xanthogranuloma in the 3rd ventricle are presented. These rare tumors can cause increased intracranial pressure and require consideration in the differential diagnosis of anterior 3rd ventricle masses.

    • The tumors obstructed interventricular foramina, causing lateral ventricle dilation and increased intracranial pressure.
    • Computerized tomography revealed dense, nonenhancing lesions in the anterior 3rd ventricle.

    Findings:

    • The described cases highlight the clinical presentation and imaging characteristics of 3rd ventricle xanthogranuloma.
    • Pathogenesis of these rare tumors is discussed.

    Implications:

    • Xanthogranuloma should be included in the differential diagnosis for anterior 3rd ventricle tumors in adults.
    • Early consideration aids in timely diagnosis and management of these rare intracranial lesions.
