    Summary

Extreme hyperimmunoglobulinemia E (HIES) causes severe staphylococcal infections and lung cysts in infants. Early treatment can resolve some cysts, but others persist, highlighting the complexity of this primary immunodeficiency syndrome.

    Area of Science:

    • Immunology
    • Pulmonology
    • Radiology

    Background:

    • Extreme hyperimmunoglobulinemia E (HIES) is a primary immunodeficiency syndrome.
    • Characterized by susceptibility to recurrent staphylococcal infections, depressed cellular immunity, and deficient antibody formation.
    • Clinical manifestations include recurrent pneumonia and cyst formation.

    Purpose of the Study:

    • To describe the radiographic course and outcomes of lung cyst formation in patients with HIES.
    • To evaluate the efficacy of antistaphylococcal therapy and surgical intervention for these cysts.

    Main Methods:

    • Radiographic analysis of 11 patients with HIES.
    • Histopathological examination of resected cysts.
    • Correlation of clinical outcomes with treatment modalities (antibiotics, surgery).

    Related Experiment Videos

    Main Results:

    • Recurrent pneumonia and cyst formation were key radiographic findings.
    • Five cysts resolved with continuous antistaphylococcal therapy.
    • Two cysts resolved post-resection; four persisted after interventions.
    • Cysts exhibited dense necrotic surfaces, fibrous walls, inflammatory infiltrates, and persistent bronchial connections.
    • Radiographic evidence of sinusitis (9/9) and mastoiditis (3/4) was also noted.

    Conclusions:

    • Lung cyst formation is a significant radiographic feature in HIES.
    • Treatment outcomes for HIES-associated lung cysts vary, with some requiring long-term management.
    • Persistent bronchial connections may contribute to cyst persistence.
    • Associated sinusitis and mastoiditis are common in these patients.
