    Summary

Continuous peritoneal lavage with a specific antibiotic solution improved survival rates in patients with peritonitis, particularly those with severe contamination. This treatment showed significant benefits without compromising anastomotic integrity.

    Area of Science:

    • Peritoneal dialysis
    • Infectious diseases
    • Surgical outcomes

    Background:

    • Peritonitis poses a significant threat to patients, especially those with poor prognoses.
    • Intraperitoneal abscesses are a common and serious complication of peritonitis.
    • Current treatments for severe peritonitis may have limitations in improving patient survival.

    Purpose of the Study:

    • To evaluate the efficacy of continuous peritoneal lavage with a combination antibiotic solution in patients with peritonitis and poor prognoses.
    • To assess the impact of this treatment on survival rates and the incidence of intraperitoneal abscesses.
    • To determine if the lavage affects anastomotic integrity.

    Main Methods:

    • A cohort of 27 patients with peritonitis and poor prognoses received continuous peritoneal lavage.
    • The lavage solution consisted of 1.5% Dianeal with gentamicin sulfate, cephalothin sodium, and lincomycin.
    • Patient survival and development of intraperitoneal abscesses were monitored.

    Main Results:

    • Twenty-one out of 27 patients (78%) survived the treatment.
    • Only two patients developed intraperitoneal abscesses.
    • The treatment demonstrated the greatest benefit in patients with gross peritoneal contamination.

    Conclusions:

    • Continuous peritoneal lavage with gentamicin, cephalothin, and lincomycin is an effective treatment for peritonitis in high-risk patients.
    • This therapeutic approach significantly improves survival rates and reduces the risk of intraperitoneal abscess formation.
    • The lavage procedure maintained anastomotic integrity, indicating its safety in surgical patients.
