    Background:

    • Smoking is a major cause of lung disease.
    • Morphologic changes in the lungs can lead to impaired lung function.
    • Early detection of these changes is crucial for managing respiratory conditions.

    Purpose of the Study:

    • To investigate the correlation between lung morphologic changes (emphysema, airway narrowing) and pulmonary function alterations in smokers.
    • To identify sensitive pulmonary function tests for detecting early lung damage.

    Main Methods:

    • Excised lung lobes from 21 smokers and 1 nonsmoker were analyzed for emphysema and bronchiolar narrowing.
    • Comprehensive pulmonary function tests were performed pre-lobectomy, including diffusing capacity, single-breath N2, elastic recoil, and flow-volume loops.
    • Morphometric measurements of emphysema and airway narrowing were correlated with pulmonary function data.

    Main Results:

    • All lobes from smokers showed some degree of abnormal pulmonary function.
    • Diffusing capacity, specifically fractional CO uptake, correlated with emphysema severity.
    • Small airway narrowing correlated with maximal mid-expiratory flow rate and the single-breath N2 test.

    • Pulmonary function tests can detect morphologic lung changes associated with smoking.
    • Diffusing capacity is a marker for emphysema, while flow rates and N2 washout indicate small airway disease.
    • Maximal flow/static recoil curves are valuable for identifying airway abnormalities in smokers with emphysema.
