    Background:

    • Gonococcal perihepatitis, a complication of pelvic inflammatory disease, often presents with symptoms mimicking acute cholecystitis.
    • Increasing incidence of gonorrhoea necessitates awareness of its varied presentations in surgical emergency settings.

    Purpose of the Study:

    • To highlight the clinical presentation and diagnosis of gonococcal perihepatitis.
    • To determine the incidence of gonococcal perihepatitis among patients presenting with suspected acute cholecystitis.

    Main Methods:

    • Retrospective review of six female patients diagnosed with gonococcal perihepatitis over a three-year period.
    • Cervix specimens were cultured for Neisseria gonorrhoeae to confirm diagnosis.
    • Clinical data, including presenting symptoms and history of pelvic infection, were analyzed.

    Main Results:

    • Six female patients presented with symptoms indistinguishable from acute cholecystitis.
    • Neisseria gonorrhoeae was identified in cervix specimens of all patients.
    • 2.6% of patients admitted with suspected acute cholecystitis were diagnosed with gonococcal perihepatitis.

    • Gonococcal perihepatitis should be considered in the differential diagnosis of acute abdominal pain in young women, especially those with risk factors for gonorrhoea.
    • Prompt diagnosis through microbiological testing is essential for effective management.
    • Increased gonorrhoea rates correlate with a higher frequency of this complication.
