    Area of Science:

    • Thoracic Surgery
    • Pulmonary Medicine
    • Surgical Pathology

    Background:

    • Severe multisegmental bilateral bronchiectasis poses significant challenges in patient management.
    • Surgical intervention for bilateral lung conditions is often approached with caution due to potential risks.

    Purpose of the Study:

    • To evaluate the long-term outcomes and safety of bilateral pulmonary resection for severe multisegmental bilateral bronchiectasis.
    • To determine if bilateral bronchiectasis is a contraindication for surgical intervention.

    Main Methods:

    • A retrospective review of 99 patients who underwent bilateral pulmonary resection for severe multisegmental bilateral bronchiectasis between 1937 and 1977.
    • Analysis of operative mortality, complication rates, and long-term symptomatic improvement following surgery.

    Related Experiment Videos

    Main Results:

    • A total of 216 operations were performed, with 20 patients undergoing three or more procedures.
    • Operative mortality was low at 1.4%, and severe complications occurred in 7% of patients.
    • 83% of patients (83 out of 99) experienced improvement in pulmonary symptoms, with a follow-up averaging 10.2 years.

    Conclusions:

    • Bilateral pulmonary resection is a viable treatment option for carefully selected patients with severe multisegmental bilateral bronchiectasis.
    • Surgical intervention can provide lasting symptomatic improvement and should not be automatically contraindicated.
