    Summary

    Constrictive albuginitis, a bilateral testicular condition causing infertility, presents with scrotal pain and firm testicles. Surgical decompression (albugineotomy) successfully treated two patients, suggesting a primary degenerative cause.

    Background:

    • Constrictive albuginitis is a rare condition affecting the tunica albuginea of the testes.
    • Sterility is a significant consequence of this condition.

    Observation:

    • Patients presented with chronic scrotal pain, small, firm testicles, and bilateral involvement.
    • Microscopic examination revealed peritubular fibrosis and degenerated spermatogenesis.

    Findings:

    • Operative findings included a thickened, rigid tunica albuginea with excessive fibrosis and hyalinosis.
    • The constrictive nature of the tunica albuginea was identified as the cause of sterility.

    Implications:

    • Decompression albugineotomy offers a successful treatment for constrictive albuginitis.
    • A primary degenerative process of the tunica albuginea is proposed as the etiology.
