Bladder neuropathy in lumbar disc disease.

C Mosdal, P Iversen, R Iversen-Hansen

    Acta Neurochirurgica
    |January 1, 1979

    Summary

Sciatica from disc lesions can cause bladder dysfunction, even without other sacral root compression signs. Many patients showed bladder hyposensibility or detrusor paresis, with two-thirds improving post-surgery.

    Area of Science:

    • Neurology
    • Urology
    • Neurosurgery

    Background:

    • Sciatica, often caused by intervertebral disc lesions, can present with bladder dysfunction.
    • These urinary disturbances may occur even when other clinical signs of sacral root compression are absent.

    Purpose of the Study:

    • To investigate bladder function in patients with lumbar disc diseases.
    • To correlate urodynamic findings with neurosurgical pathology and surgical outcomes.

    Main Methods:

    • Urodynamic examination, including water cystometry and pressure/flow studies, was performed preoperatively in 32 patients.
    • Anal sphincter surface electromyography (EMG) was also recorded simultaneously.
    • Micturition symptoms were registered and bladder function assessed.

    Main Results:

    • Nearly half of the patients exhibited bladder hyposensibility (delayed first sensation).
    • A quarter of patients demonstrated detrusor paresis, voiding primarily by straining.
    • These bladder function abnormalities were not directly related to the specific neurosurgical pathology.

    Conclusions:

    • Lumbar disc disease can lead to significant bladder dysfunction, including hyposensibility and detrusor paresis.
    • While some patients show improvement post-surgery, a notable portion may have persistent bladder issues.
    • Further evaluation is recommended for patients with pronounced symptoms or abnormal preoperative urodynamics.
