    Summary

Combination chemotherapy shows promise for advanced diffuse histiocytic lymphoma (reticulum-cell sarcoma). Over 40% of patients achieved complete remission, with most remaining disease-free for years, suggesting a potential cure.

    Area of Science:

    • Oncology
    • Hematology
    • Pharmacology

    Background:

    • Advanced diffuse histiocytic lymphoma (DHL), also known as reticulum-cell sarcoma, is an aggressive hematologic malignancy.
    • Historically, DHL has a poor prognosis with rapid progression and high fatality rates when treatment is unsuccessful.

    Purpose of the Study:

    • To evaluate the efficacy of a specific combination chemotherapy regimen in patients with advanced diffuse histiocytic lymphoma.
    • To assess long-term remission and survival rates following this treatment approach.

    Main Methods:

    • A cohort of 27 patients with advanced DHL received combination chemotherapy.
    • The regimen included nitrogen mustard (or cyclophosphamide), procarbazine, vincristine, and prednisone (a CHOP-like regimen).

    Related Experiment Videos

    Main Results:

    • Eleven patients (41%) achieved a complete remission (CR).
    • Of those in CR, only one experienced tumor recurrence.
    • The remaining ten CR patients remained free of disease for 26-105 months post-treatment.
    • All patients who did not achieve CR or had only a partial response (PR) unfortunately died.

    Conclusions:

    • Combination chemotherapy, including nitrogen mustard/cyclophosphamide, procarbazine, vincristine, and prednisone, is effective in achieving durable complete remissions in advanced DHL.
    • Survival free of disease exceeding 2 years post-treatment may be considered a cure for DHL, similar to radiotherapy outcomes.
    • This treatment regimen offers a significant survival benefit for patients with advanced diffuse histiocytic lymphoma.
