Anatomical effects of sudden middle ear pressure changes.

    Summary

Brief middle ear pressure changes in guinea pigs caused acute ear damage. Negative pressure more frequently caused tympanic membrane perforations and hemorrhage than positive pressure.

    Area of Science:

    • Otolaryngology
    • Auditory Physiology
    • Experimental Otology

    Background:

    • Middle ear pressure fluctuations can affect inner ear structures.
    • Understanding these effects is crucial for diagnosing and treating ear conditions.

    Purpose of the Study:

    • To investigate acute middle and inner ear alterations in guinea pigs following controlled middle ear pressure changes.
    • To correlate the magnitude and direction of pressure with observed pathological findings.

    Main Methods:

    • Guinea pigs were subjected to brief, unilateral, phasic middle ear pressure changes (up to +/- 6000/mm H2O).
    • Otic structures were examined for pathological changes.
    • Effects of pressure magnitude and direction were analyzed.

    Main Results:

    • Middle ear findings included tympanic membrane perforations, exudates, and hemorrhage, related to pressure magnitude.
    • Negative pressure more commonly caused perforations and hemorrhage than positive pressure.
    • Positive pressure caused air bubbles behind the round window; hemorrhage occurred in scala tympani and scala vestibuli.

    Conclusions:

    • Acute middle ear pressure changes induce significant middle and inner ear damage in guinea pigs.
    • The extent and type of damage are dependent on the magnitude and direction of the applied pressure.
    • Contralateral ear changes were also observed, suggesting a potential systemic effect.
