    Background:

    • Cataract is a leading cause of vision loss.
    • The role of immune response in cataract progression is not fully understood.
    • Lens crystallins are potential autoantigens.

    Purpose of the Study:

    • To investigate immune system sensitization to lens crystallins in cataract patients.
    • To compare immune responses before and after different types of cataract surgery.

    Main Methods:

    • Lymphocyte stimulation test was performed on two groups of cataract patients.
    • Antigens used included human (total) lens crystallins and bovine alpha crystallin.
    • Patients were tested before and after cataract surgery (extracapsular vs. intracapsular extraction).

    Main Results:

    • 38% of pre-operative patients showed positive response to lens crystallins, 15% to alpha crystallin.
    • Extracapsular lens extraction increased positive responses compared to non-operated patients.
    • Intracapsular lens extraction resulted in less sensitization than extracapsular extraction.

    Conclusions:

    • Cataract progression may involve leakage of lens antigens other than alpha crystallin.
    • Cataract surgery, particularly extracapsular extraction, can enhance immune sensitization to lens crystallins.
    • Further research is needed to understand the clinical implications of this immune response.
