    Summary

    Cystic meningiomas can present with large xanthochromic cerebral cysts, mimicking glial tumors on CT scans. Definitive radiological differentiation is challenging, underscoring the need for biopsies of suspected brain neoplasms.

    Background:

    • Meningiomas are common primary intracranial tumors.
    • Cystic changes within or adjacent to meningiomas can occur.
    • Distinguishing cystic meningiomas from other brain tumors is clinically important.

    Observation:

    • Presents three cases of cystic meningioma.
    • Cysts associated with meningiomas can be central, peripheral, or in adjacent brain tissue.
    • Computed Axial Tomography (CAT) scan appearances can mimic glial tumors with cystic or necrotic changes.

    Findings:

    • Cystic meningiomas may be misdiagnosed preoperatively due to similar imaging characteristics as glial tumors.
    • Gross surgical appearance can also resemble malignant glioma.
    • No definitive radiological features reliably differentiate cystic meningiomas from malignant gliomas.

    Implications:

    • Highlights the diagnostic challenge posed by cystic meningiomas.
    • Emphasizes the critical need for biopsy in diagnosing suspected cerebral neoplasms.
    • Suggests that radiological appearance alone is insufficient for definitive diagnosis.
