    Summary

    Priapism, a prolonged erection, occurred in two leukapheresis procedures. This may be linked to enhanced granulocyte aggregation and heparin-induced anti-platelet antibodies during these medical treatments.

    Background:

    • Filtration leukapheresis and hemodialysis are medical procedures involving blood processing.
    • Both procedures can activate the complement system, specifically generating C5a.
    • Heparin administration is common in both procedures and can induce immune responses.

    Observation:

    • Priapism, a rare complication, was observed in 2 out of 3,680 filtration leukapheresis procedures.
    • Priapism has also been reported in patients undergoing hemodialysis.

    Findings:

    • Enhanced granulocyte adhesion and aggregation, potentially mediated by C5a, are common to both procedures.
    • Heparin can induce heparin-dependent anti-platelet antibodies, suggesting an immune mechanism.
    • Complement-mediated venous leukostasis or immune-induced platelet aggregates may obstruct penile blood flow, causing priapism.

    Implications:

    • Understanding these mechanisms can help mitigate the risk of priapism during leukapheresis and hemodialysis.
    • Further research into complement activation and heparin-induced antibodies is warranted.
    • This finding may inform patient management and procedural protocols in nephrology and transfusion medicine.
