    Summary

    Sarcoidosis can affect the heart, causing pericardial effusions. Echocardiography revealed small effusions in 19% of sarcoidosis patients, highlighting cardiac involvement in this systemic inflammatory disease.

    Background:

    • Sarcoidosis is a multisystem granulomatous disease of unknown etiology.
    • Cardiac involvement in sarcoidosis can manifest in various forms, including pericardial disease.
    • Pericardial effusion is a recognized but less common manifestation of sarcoidosis.

    Observation:

    • A case of sarcoid pericarditis with a transudative pericardial effusion and low complement level is presented.
    • Echocardiographic screening for pericardial effusions was performed in 48 consecutive patients with confirmed sarcoidosis.
    • A review of ten previously reported cases of symptomatic parietal pericardial sarcoidosis was conducted.

    Findings:

    • Small pericardial effusions were detected in 19% of patients with sarcoidosis.
    • The pericardial effusion in the presented case was characterized as a transudate with low complement levels.
    • The review of existing literature identified ten cases of symptomatic parietal pericardial sarcoidosis.

    Implications:

    • Echocardiography may be a useful tool for detecting subclinical pericardial effusions in sarcoidosis patients.
    • Understanding the prevalence and characteristics of cardiac involvement is crucial for comprehensive sarcoidosis management.
    • Further research is warranted to elucidate the mechanisms and clinical significance of pericardial involvement in sarcoidosis.
