    Rickettsialpox is rarely reported, with only one case annually in recent years. However, a New York City case suggests clinicians may underdiagnose this uncommon infectious disease.

    Background:

    • Rickettsialpox is an uncommon infectious disease caused by Rickettsia akari.
    • Historically, rickettsialpox has been infrequently reported in the United States.

    Observation:

    • A case of rickettsialpox was identified in New York City.
    • Review of New York City Health Department records revealed only six serum samples submitted for rickettsialpox testing in the past decade.

    Findings:

    • The low number of reported cases and limited serological testing suggest rickettsialpox may be underdiagnosed.
    • Clinicians might be failing to recognize the clinical, diagnostic, and epidemiologic features of rickettsialpox.

    Implications:

    • Increased awareness and diagnostic vigilance among healthcare providers are necessary.
    • Further epidemiological studies are warranted to determine the true incidence of rickettsialpox.
    • Improved recognition could lead to more accurate public health surveillance of this infectious disease.
