    Summary

    This study found that aspirin use in rheumatoid arthritis patients rarely causes kidney damage. Even with long-term use, renal function remained largely unaffected, with normal creatinine levels observed.

    Background:

    • Rheumatoid arthritis (RA) patients often use aspirin for pain relief.
    • Concerns exist regarding long-term aspirin use and potential kidney damage (analgesic nephropathy).

    Purpose of the Study:

    • To investigate the impact of long-term aspirin consumption on renal function in rheumatoid arthritis patients.
    • To determine if aspirin dose, rate, or duration of ingestion correlates with impaired kidney function.

    Main Methods:

    • Renal function was assessed in 17 RA patients with a history of significant aspirin consumption (5-20 kg).
    • 14 patients were re-evaluated after two years, with some consuming additional aspirin.
    • Plasma creatinine levels and creatinine clearance were measured.

    Main Results:

    • No significant relationship was found between aspirin intake (total dose, rate, or duration) and measured aspects of renal function.
    • All patients maintained plasma creatinine levels within the normal range.
    • A slight depression in creatinine clearance was observed in some patients, but not consistently linked to aspirin dose.

    Conclusions:

    • Aspirin, when prescribed alone at appropriate doses for rheumatoid arthritis, rarely causes analgesic nephropathy.
    • Long-term aspirin use in RA patients, within the studied parameters, does not appear to lead to significant renal impairment.
