    Summary

    Serum alpha-fetoprotein (AFP) levels can be elevated in non-cancerous liver diseases, suggesting hepatic regeneration. Sensitive assays reveal higher AFP in hepatitis patients compared to those with benign non-hepatic conditions.

    Background:

    • Serum alpha-fetoprotein (AFP) is a biomarker often associated with hepatocellular carcinoma.
    • Elevated AFP levels in non-neoplastic liver conditions require further investigation.

    Purpose of the Study:

    • To evaluate serum AFP levels in patients with biopsy-proven non-neoplastic hepatic disorders.
    • To compare AFP levels in hepatic disorders with those in non-hepatic benign diseases.

    Main Methods:

    • Radioimmunoassay was used to measure serum AFP levels.
    • Study included 473 patients with non-neoplastic hepatic disorders and 350 patients with non-hepatic benign diseases.

    Main Results:

    • 22% of patients with non-neoplastic hepatic disorders had AFP > 40 ng/ml, versus 0.3% in non-hepatic benign diseases.
    • Elevated AFP levels were observed in hepatitis, cirrhosis, and biliary tract obstruction.
    • Very high AFP levels (>3000 ng/ml) were rare, primarily seen in subacute hepatic necrosis and hepatoma.

    Conclusions:

    • Sensitive assays show elevated serum AFP in various non-neoplastic liver conditions, potentially indicating hepatic regeneration.
    • AFP elevation in non-neoplastic liver disease necessitates careful interpretation to distinguish from malignancy.
