    Summary

    Patients with Idiopathic Addison's disease (IAD) may have early thyroidal insufficiency. Autoimmune thyroid damage is suggested by elevated thyroid microsomal antibodies and altered thyroid hormone levels in euthyroid individuals.

    Background:

    • Idiopathic Addison's disease (IAD) is an autoimmune condition.
    • Subclinical hypothyroidism can precede overt thyroid dysfunction.
    • Assessing thyroid function in IAD patients is crucial.

    Purpose of the Study:

    • To investigate the prevalence of subclinical hypothyroidism in patients with IAD.
    • To explore the relationship between autoimmune markers and thyroid function in IAD.

    Main Methods:

    • Studied 14 clinically euthyroid IAD patients.
    • Measured serum thyroxine (T4), triiodothyronine (T3), and thyroid-stimulating hormone (TSH).
    • Assessed thyroid microsomal antibodies and TSH response to bovine thyrotrophin.

    Main Results:

    • 11/14 patients had thyroid microsomal antibodies.
    • Mean basal TSH was elevated, though only 3 patients exceeded the normal range.
    • Decreased serum T4 and elevated serum 3,3',5'-triiodothyronine were observed.
    • Higher TSH and antibody titers correlated with reduced T3 release.

    Conclusions:

    • Euthyroid IAD patients may exhibit early thyroidal insufficiency.
    • Progressive autoimmune thyroid damage is a likely cause.
    • Subclinical thyroid dysfunction should be considered in IAD management.
