    Background:

    • Bipolar I disorder is a severe mood disorder characterized by manic and depressive episodes.
    • A subset of patients present with mania without a history of depressive episodes, termed "unipolar mania".

    Purpose of the Study:

    • To investigate the clinical characteristics and treatment response of "unipolar mania" within bipolar I disorder.
    • To determine if "unipolar mania" represents a distinct diagnostic entity.

    Main Methods:

    • Retrospective analysis of 241 patients with bipolar I disorder at a lithium clinic.
    • Comparison of clinical features (hospitalization, somatic treatment, rapid cycling, suicide attempts, family history) between "unipolar manic" and other bipolar I patients.
    • Assessment of lithium's prophylactic efficacy in the "unipolar manic" group.

    Main Results:

    • 15.7% of bipolar I patients met criteria for "unipolar mania" (no prior depression hospitalization/treatment).
    • "Unipolar manic" patients had significantly lower rates of rapid cycling and suicide attempts compared to other bipolar I patients.
    • No differences in the risk of illness in first-degree relatives were observed. Lithium was effective as a prophylactic agent.

    Conclusions:

    • "Unipolar mania" appears to be a subgroup within bipolar I disorder, not a separate illness.
    • This subgroup exhibits distinct clinical features and responds well to lithium prophylaxis.
    • Further research is needed to fully elucidate the nosological status of "unipolar mania".
